Take a scorpion, or a poisonous spider.
Impale it and let it dry.
Pound it up into powder and sprinkle it on the intended victim's food.
In time, live scorpions will grow in the body, and not only will they cause discomfort by their very presence, but they also will secrete their poisonous venom into the victims veins.

Get some tapeworms put them inside a fish.
Fry the fish and serve it to the victim.
In three days, the victim's intestines will be rife with worms.
